VSCDA Medical Qualifications Policy:
You must have a current medical (Physical Exam Form) on file with the VSCDA office. Physicals are valid for two years from date of doctor’s certification.
The Physical Exam Form consists of two pages. The driver/applicant completes and certifies the first page, which consists of your medical history. The doctor completes and certifies the second page, which is the actual physical examination report. The form set also includes a letter to your doctor explaining the nature of your involvement in Vintage Racing and the standards we’d like used to evaluate your physical condition.
Your medical qualifications are not complete unless VSCDA has both certification pages on file.
Drivers over age 60 please note: If you are planning to apply for and maintain a Vintage Motorsports Council Competition License, you are required to have a current medical form on file with the VSCDA office which is updated yearly. VMC requirements state that physicals for drivers over 60 are valid for one year from date of doctor’s certification. VSCDA is the certifying agency for your application to VMC for medical qualifications and must be able to verify satisfaction of this requirement to them as part of your application.
Please download and print the forms for use as you need them using the links above.
